29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers: On or about September 22, 2014, at the worksite located at 7426 Cantrell Road in Little Rock, Arkansas, the open trench in the Service Bay area was not guarded and subcontractor employees were not protected from inadvertently falling into the trench. Employees were exposed to falls or greater than 6 ft. into the bottom of the trench.
