The documentation did not identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test.      a) On piping of the covered process, on or about 9/24/14: Documentation of annual thickness measurements taken on process piping did not identify the specific location on pipes where the measurement was taken.      b) On the R3 reactor, on or about 9/24/14: Thickness measurements taken on the R3 reactor were not documented.      Abatement certification must be submitted for these items.
