29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about September 22, 2014, at the worksite located at 7426 Cantrell Road in Little Rock, Arkansas, wooden boards, pipe pieces and other miscellaneous debris were scattered on the ground on the south side of the building being constructed. This condition exposed employees to trips, slips and falls from the same elevation.

Employees were exposed to falls of greater than 6 ft. into the bottom of the trench.
