29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    On or about September 22, 2014, at the worksite located at 7426 Cantrell Road in Little Rock, Arkansas, wooden boards, pipe pieces and other miscellaneous debris were scattered on the ground on the south side of the building being constructed.  This condition exposed employees to trips, slips and falls from the same elevation.

29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:    On or September 22, 2014, at the worksite located at 7426 Cantrell Road in Little Rock, Arkansas, the open trench in the Service Bay area was not guarded and employees were not protected from inadvertently falling into the trench.  Employees were exposed to falls of greater than 6 ft. into the bottom of the trench.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or warning line system and safety monitoring system:    On or about September 22, 2014, at the worksite located at 7426 Cantrell Road in Little Rock, Arkansas, employees on a 4:12 pitched roof engaged in roof decking activities were exposed to unprotected falls of greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about September 22, 2014, at the worksite located at 7426 Cantrell Road in Little Rock, Arkansas, the portable ladder located on the south side of the building used to access and descend from the roof was placed beneath the roof overhang and did not extend 3 feet above the landing surface.  This condition exposed employees to falls from elevations.
