29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).    Worksite: The employee who was manually receiving materials while standing on the open edge of the second floor of the residential house under construction was not protected from 13 feet fall hazards by the use of guardrails, safety nets or a personal fall arrest system.      On or about 9/22/2014, the employee lost balance and fell 13 feet 1 inch to the lower floor and sustained severe injuries from the incident.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Worksite: The employee who was exposed fall hazards of up to 28.5 feet above the ground while working on 6 to 8 pitched roofs was not trained in fall hazards and the procedures to be followed to minimize such hazards.
