29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: At the above addressed jobsite, employees working below commercial construction operations were not wearing hard hats to protect against falling material and debris. Employees were exposed to struck-by hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e): At the above addressed jobsite, employees working from JLG 600AJ Boom Lifts were not wearing a body belt or full body harness. The employees were thereby exposed to the hazards associated with a fall of greater than twenty (20) feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
