29 CFR 1926.502(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking/working level: (a) MDS Drywall Specialists, Inc. at 1600 Ironhorse Dr, Longmont, CO 80501: On or before September 2, 2014 the exposing employer did not ensure a midrail was installed midway between the top rail and the walking/working surface on a third level balcony of building G. An employee was cutting drywall on the balcony. This condition exposed the employee to a fall of approximately 15 feet.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided: (a) MDS Drywall Specialists, Inc. at 1600 Ironhorse Dr, Longmont, CO 80501: On or before September 2, 2014 the exposing employer did not ensure that employees had proper access to the outside stairways at building F, in that there was a break in elevation of 29 inches from the landing to the ground level. This condition exposed employees to trip hazards.
