OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using an automotive lift:    (a)  On or about September 19, 2014, and at times prior the employer did not ensure that employees         were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:        (1)  Benwil lift, model GPO-7, serial #B89T0003, did not have the locking mechanism locked             into place while the lift was raised and in use.        (2)  Benwil lift, model SPOA10N5G0 Serial #CNP14D0075, did not have the locking mechanism locked             into place while the lift was raised and in use.        (3)  Benwil lift, model SPOA10N5G0 Serial #CNP14F0015, did not have the locking mechanism locked             into place while the lift was raised and in use.        (4)  Benwil lift, Model GPO-7, serial #B8809054, was available for use and not taken out of service.  The lift exhibited            defects such as, but not limited to, the raising and lowering lever was broken and the lock could not engage            without manual intervention and other components (two springs) were not correctly affixed.          (5)  The employer did not document that all lift users were trained to use the lifts in that several rotary lifts were not              lowered onto locks while in use.
