The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) The company did not develop and implement an energy control procedures, training and periodic reviews to address control of hazardous energy while performing servicing and maintenance of equipment such as but not limited to the printing press.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about September 23, 2014 the electrical disconnect on the Miehle 7/c printing press was not shut off while cleaning the rollers, exposing employee to nip points and/or moving parts.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. a) On or about September 23, 2014 a lockout device was not attached to the energy isolating point on the Miehle 7/c pressing press during the cleaning process of the rollers, exposing employee to pinch points.
