29 CFR 1910.23(a)(4):  Every skylight floor opening and hole shall be guarded by a standard skylight screen or a fixed standard railing on all exposed sides.  a.  On the roof of the facility, on or about 09/26/14:  Three skylights were not guarded by a screen or standard guardrail on every exposed side.  Abatement certification must be submitted for this item.

29 CFR 1910.1030(h)(1)(ii)(B): The employer did not establish and maintain an accurate record for each employee with occupational exposure which includes a copy of the employee's hepatitis B vaccinations and any medical records relative to the employee's ability to receive vaccination as required by paragraph (f)(2) of the OSHA Bloodborne Pathogens Standard.  a.  Throughout the facility, on or about 09/26/14:  The employer, having employees having occupational exposure to blood and/or other potentially infectious material (OPIM), did not maintain an accurate record of each employees hepatitis B vaccination status due to conflicting information contained in the employees medical record files.  Some records showed employees had previously received proof of the completed Hepatitis B vaccination series however, employee shot records were not obtained and titers were not taken.   Abatement certification must be submitted for this item.
