OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to entrapment hazards:     Employees were exposed to entrapment hazards while operating a vacuum hose with negative pressure. On or about 8-26-14 a worker's arm was caught in the hose of a vacuum truck.    Methods of abatement include but are not limited to:    1) Follow manufacturer's recommendations and install an inline relief valve near the end of the hose;  2) Re-train all members of the vacuum truck crews on all the methods of reducing or stopping the vacuum pressure:  3) Develop daily safety inspection checklist for VacTruck;  4) Conduct daily safety inspections of the vacuum trucks and correct safety issues.
