29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:      On or about September 24, 2014, and at times prior thereto, employees were exposed to the chemical/health hazard of lead while wearing a North 1100-30M, half face respirator with P100 filter cartridges without a medical evaluation or fit testing to ensure the employee could properly wear the respirator and that it would not cause any adverse health effects.
