(a) Location - 165 Cambridge Place, Cambridge, MA (Fourth Floor): On or about 9/24/2014, a job made platform not meeting the design criteria established by industry consensus was used in conjunction with a rough terrain forklift during the unloading of doors through a sliding glass door.
