29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A: (a) Workplace - The employer had not maintained a record verifying that an employee, at risk of exposure to equipment contaminated with blood and or other pathogens while performing endoscope reprocessing, had been provided a Hepatitis B declination statement, on or about September 23, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
