29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: Road Maintenance Shop- (a) The heads of the metal punches were mushroomed and not dressed to prevent flying metal debris from striking employees. (b) The handle of the hammer was cracked and exposed employees to injury from a loose hammer head. (c) The Milwaukie grinders abrasive wheel was loaded with metal potential causing the wheel to heat up and break apart, exposing the employees to flying debris. Bureau of Land Management was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 242(a), which was contained in OSHA Inspection Number 950980, Citation Number 1, Item Number 1, and was affirmed as a final order on 03/21/2014 with respect to a work place located at, Molalla, OR and contained in OSHA Inspection Number 331945, Citation Number 2, Item Number 2, and was affirmed as a final order on 09/28/2012 with respect to a work place located at North Bend, OR. Note: Abatement certification and supporting documentation are required for these items.
