29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: At a jobsite in Stillman Valley, Illinois, excavation of soil around the base of a utility pole during the sloping of a trench compromised the stability of an electrical utility pole that ran parallel to the trench without any support provided to prevent it form collapsing. Employees were exposed to potential injury from crushing and engulfment. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: At a jobsite in Stillman Valley, Illinois employees were not protected from cave-in hazards while accessing and egressing a trench that was approximately 20 feet below grade. Employees were exposed to potential injury from crushing and engulfment. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
