29 CFR 1910.178(m)(10): A load backrest extension shall be used whenever necessary to minimize the possibility of the load or part of it from falling rearward. Warehouse - On or about September 26, 2014, the Toyota sit down forklift truck was not equipped with a load backrest extension. Employees were thereby exposed to materials falling rearward.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. Warehouse - On or about September 26, 2014, the battery connectors on two Toyota walkers were defective in that wires were frayed and not tight. Employees were thereby exposed to the hazards associated with exposed energized wires.
