The platform was constructed with two planks, leaving the remaining area unplanked; on, or about, 09/26/2014. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

A rope was used to tie the eastern most side of the top scaffold section to a post on the third story of a residential building; on, or about, 09/26/2014. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
