29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees, when the employer had not satisfied the requirements of 1926.451(f)(15)(i), (ii), (iii) and (iv):    a) 789 Saint Georges Avenue, Woodbridge, NJ 07095: The employee was exposed to a fall hazard over 20 feet to the concrete sidewalk below, when he was working on and from an extended portable aluminum ladder placed inside the cage of the scissor lift to extend his reach while spray painting newly placed stucco.   Condition occurred on or about 09/26/14.
