OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to the hazard of being crushed or run over by field trucks:      a) Potato storage area near intersection of Shelley West Road, 475 E. Hayes Project Road & 1300 N. Horseshoe Road: On or about September 23, 2014, and at times prior thereto, employees were backing field trucks with large blind spots to unloading conveyors during low light conditions in the night and early morning while employees were working on the ground around the field trucks.  The reverse lights for the Freightliner Model FL 70 field truck were not functioning.    Note: Abatement certification AND documentation are required for this item.
