29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. On or about September 25, 2014, front of building, The employer permitted an employee to work in an extensible boom platform that was within 10 feet of an unguarded energized power line (7,200 volts), which the employee could contact during the course of building demolition and exposes employees to electrical hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1925.502(e). a. On or about September 25, 2014, front of building, a fall protection harness and lanyard was not worn by an employee working in an extensible boom platform for building demolition work and the employee was exposed to fall hazards.

29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations: a. On or about September 25, 2014, front of building, prior to permitting employees to start building demolition the employer did not have in writing evidence of a building survey completed by a competent person to determine the condition of the framing, floors, and walls and the possibility of unplanned collapse. Employees were exposed to struck-by hazards during building demolition operations.
