29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: This violation occurred on or about September 29, 2014 at a jobsite located at 1041 Enchanted Rock in Allen, Texas; where at least three (3) employees were not provided with a portable or hook on ladder and were using the end frames and cross braces to gain access to the upper levels of the two tier tubular welded frame scaffold.
