29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:     a) For employees working at 2356 West Hillsborough Avenue in Tampa, as observed on or about September 23, 2014. Employees were using a fabricated platform attached to the forklift to work on the sides of the building. The forklift manufacturer notice sticker in the cab of the forklift states the machine is not equipped to lift personnel.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a) For employees working at 2356 West Hillsborough Avenue in Tampa, as observed on or about September 23, 2014. Employees were installing plywood sheathing on the upper working surface. The edge to ground height measured approximately 23 feet 4 inches. The employees were not using fall protection or an alternative means of protection while installing the plywood sheathing.
