29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. Southeast - On or about September 29, 2014, the passageway to the southeast exit door was obstructed by production equipment, conveyor belts, and wood pallets with merchandise, thereby egress is not readily accessible.

29 CFR 1910.178(m)(10): A load backrest extension shall be used whenever necessary to minimize the possibility of the load or part of it from falling rearward. Production area - On or about September 29, 2014, the Caterpillar #29 ( serial # 4SM01861), Nissan #3 (serial # 9C31157) and Toyota #12 (serial # 63207) forklift trucks were operated with no load backrest extension. Employees were thereby exposed to falling materials.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. Production area - On or about September 29, 2014, the battery connecter on the Raymond rider was defective in that wires were frayed and not tight. Employees were thereby exposed to the hazards associated with exposed engerized wires.
