29 CFR 1926.20 (b)(2) The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer. On or about September 29, 2014, and at times prior thereto, the employer did not perform frequent and regular inspections for safety hazards such as but not limited to; fall hazards, eye protection and head protection. Employees were exposed to these and other hazards at the property under construction.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: On or about September 29, 2014, employees worked from an elevated Arrow personnel platform that was not securely attached to the lift to prevent the platform from sliding off. The platform was used with a Terex Handlers TH844C rough terrain forklift, exposing employees to a fall hazard of approximately 12 feet.
