29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:  a)    On September 18, 2013, employees in the 2nd shift working in the warehouse at 101 S. 16th St, Manitowoc WI, were installing platforms on the wind towers.  They had added the telescoping lift; the adaptor plate and ball attachment; and the top positioning platform to the Toyota Forklift Model #8FGU25.  As the operator started lifting the forks with attachments and the platform, the aluminum ball attachment snapped off the adaptor plate and the load fell to the ground.  Employees were thereby exposed to being struck by the falling forks, attached end equipment, and load.
