29 CFR 1926.1101(k)(2)(i): Before work subject to this standard was begun, building and facility owners did not  determine the presence, location, and quantity of ACM and/or PACM at the work site pursuant to paragraph (k)(1) of this section:    a)  On or about August 28, 2014, and at times prior, employees were exposed to asbestos fibers because the employer failed to determine the presence, location and quantity of presumed asbestos containing material (PACM), such as wall and ceiling surfacing material, that was occasionally disturbed by maintenance and contractor employees and failed to provide such information to the employees working in the building.
