29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative:   a)  Ross Hall, Room P-2 - On or about 05/20/2014, an electrician was injured when 30 kz feeders entering a switchgear which were believed to be de-energized were not rendered inoperative, in that, the feeders unexpectedly re-energized while the electrician was performing repairs to cables within the switchgear.
