29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  On or about October 01, 2014, and at times prior thereto, the employees were not trained by a competent person to recognize the hazards associated with erecting pump scaffolding systems, exposing employees to a fall hazard of approximately 12 feet.

29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  On or about October 01, 2014, the employer did not ensure that the pump jack scaffold system installed on the west side of the property under construction had guardrails installed, exposing employees to a fall hazard of approximately 12 feet.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage.   On or about October 01, 2014, the scaffolding system installed on the west side of the property under construction was supported only by lumber, exposing employees to the hazards of falls in the event of scaffold failure or slippage.

On or about October 01, 2014, the pump jack scaffold system set up on the west side of the property under construction was supported by lumber instead of components approved by the manufacturer, exposing employees to the hazards of falls in the event of scaffold collapse or failure.
