Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage. On or about October 01, 2014, the scaffolding system installed on the west side of the property under construction was supported only by lumber, exposing employees to the hazards of falls in the event of scaffold failure or slippage.
