29 CFR 1910.67(c)(2)(ii): An employee(s) operating an aerial lift(s) was not properly trained:  a)Worksite, Red Light Camera - An employee repairing a camera system from an Altec Model # AT200A, Serial # 0311BV16692 bucket truck, approximately 12 feet above the ground, was not wearing a body belt or harness attached to the boom or basket. The employee did not recognize the hazard and means to correct the hazard, the employee was not trained; on or about 10/1/2014.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a)Worksite, Red Light Camera - An employee repairing a camera system from an Altec Model # AT200A, Serial # 0311BV16692 bucket truck, approximately 12 feet above the ground, was not wearing a body belt or harness attached to the boom or basket; on or about 10/1/2014.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1910.135(a)(2): The employer did not ensure that a protective helmet designed to reduce electrical shock hazard was worn by each affected employee when near exposed electrical conductors which could contact the head:  a) worksite, Red Light Camera - An employee was repairing a red light directly underneath the approximate 220 volt secondary lines; on or about 10/1/2014.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
