29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about October 1, 2014, at the north side of the roof, three employees using pneumatic nailers to perform bracing work and install a fascia were not wearing eye protection, exposing themselves to being struck by flying debris or nails.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a. On or about October 1, 2014, two employees installing a fascia on a 5:12 pitch roof framing were exposed to a 19 ft. 6 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system. b. On or about October 1, 2014, five employees engaged in bracing work on a 5:12 pitch roof framing were exposed to a 17 ft. fall hazard, and up to 21 ft. 6 in., without the use of personal fall arrest system, safety net system of guardrail system.
