29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) On October 1, 2014 an employee performed framing activities without a fall protection system in place.    b) On October 1, 2014 an employee performing framing activities wore a fall protection harness that was not attached to a lanyard or an anchor point.      Note) Certification of corrective action IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    a) On October 1, 2014 the employer and the employees did not recognize the hazard of performing framing activities on a low sloped roof without an appropriate and effective fall protection system in place.  The appropriate precautions were not taken to eliminate or minimize the hazard.      Note) Certification of corrective action IS required for this item.
