29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: On the roof of a residential home, North side, on or about October 1, 2014, employees without fall protection were exposed to a fall hazard greater than 6 feet to the unpaved ground below, while installing shingles on a roof with a pitch of 6:12. Felipe Chavez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 966038, citation number 1, item number 1 and was affirmed as a final order on 5/01/14, with respect to a workplace located at 6842 Fort Bend San Antonio, TX 78222.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At a new residential construction site, West side, on or about October 1, 2014, employees were using a ladder that did not have the side rails at least 3 feet past the roof landing; to access and install asphalt shingles on a roof with a pitch of 6:12.
