29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    On or about September 11, 2014 and times prior thereto, employees of Industrial Roofing and Construction, LLC performed construction activities on the rooftop of the Georgia-Pacific, LLC facility located at or near 100 Mill Supply Road in Crossett, Arkansas.  Employees repairing the skylights with plastic wrap were not protected from tripping on or steeping into the skylights, exposing employees to fall hazards greater than 20 feet to the concrete floor.
