Factory Site: On or about October 1, 2014, and at times prior thereto, the employer did not maintain a dry floor when cleaning the roasting and packaging machines while the forklifts were operating. This condition exposed the employees to struck-by hazards.

This condition exposed the employee to a fall greater that 4-feet.
