On 10/1/14, in the Danielsville location, employees were exposed to crushing hazards as the employer did not ensure hoists used at the facility were constructed and maintained in a safe way. Hoists were not inspected and there have been incidents of hoists falling from overhead rails, safety latches are missing/broken on hooks, chains used are not inspected, hoists are routinely overloaded and spreader plates used with the hoists have not been rated or marked.
