29 CFR 1926.501(b)(10): Each employee engaged in roofing on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:  On or about October 1, 2014, and times prior thereto, employees who were installing a new roof at an existing commercial building were working outside the warning line system and exposed to an unprotected fall hazard of a height over 6 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about October 1, 2014, and times prior thereto, the employer did not ensure employees engaged in roofing activities were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize those hazards.
