29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  The employer did not develop and implement a program to address the control of hazardous energy, including but not limited to information and training, outside servicing personnel, and group lockout.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    (a)  On or about September 26, 2014, the employer did not provide employees energy control hardware, such as but not limited to locks, tags, and hasps, for maintenance and servicing a conveyor belt at 6101 N. 64th Street in Milwaukee, Wisconsin.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:      (a)  Training on lockout/tagout at the authorized level was not accomplished for employees who perform maintenance and servicing work which requires the use of energy control.
