29 CFR 1910.176(b): Storage of material created a hazard:    A)    United Lily Ann Cabinets, LLC, located at 1630 Coining Drive, Toledo, OH 43612: On or about October 1, 2014, the employer did not ensure that the shelving units, exposed to powered industrial vehicle's traffic in the warehouse area, were secured to the floor/wall from being tipped over if struck by a powered industrial vehicle. Employees, working in the area, were exposed to struck-by hazards from parts on the shelves and the shelving itself.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
