29 CFR 1926.1052(a)(4): A platform was not provided where a door or a gate opened directly on a stairway: a) On or about August 17, 2014, location was on Barge #20, on the original steps leading from the Break Shack, there was no platform. The door opened directly into the stairs. b) On Barge #20, on the Orange Container steps, the steps leading from the container, there was no platform. The door opened directly into the stairs.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: a) On or about August 17, 2014, location was on Barge #20, for the original steps leading into the Break Shack, the stairs only had one stair rail on the left side leading into the shack. b) On Barge #20, for the newly constructed steps leading into the Break Shack, the stairs only had one stair rail. The platform measured 30.5 inches from the barge deck. c) On Barge #20, on the Orange Container steps, only one stair rail was installed. The floor in the container measured 30.5 inches from the barge deck.
