29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  The employer does not protect each employee on a scaffold more than 10 feet above a lower level from falling. This violation was observed on or about October 2, 2014, on the west side of the building where employees were exposed to a fall hazard of 13 feet when applying stucco siding from fabricated frame scaffolding without being protected from falling to the lower level.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee on a scaffold more than 10 feet above a lower level are protected from falling to that level.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway.  The employer does not install guardrail systems along at least one side of walkways within a scaffold. This violation was observed on or about October 2, 2014, on the west side of the building where employees were exposed to a fall hazard of 13 feet when applying stucco siding from fabricated frame scaffolding without the walkways having a guardrail system installed along at least one side.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee on a walkway located within a scaffold is protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  The employer does not protect each employee on a scaffold with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. This violation was observed on or about October 2, 2014, on the west side of the building where employees were exposed to a struck-by hazard of when applying stucco siding from two levels of fabricated frame scaffolding without being provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling object.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee on a scaffold is provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.

This violation was observed on or about October 2, 2014, on the west side of the building where employees were exposed to a fall hazard of 13 feet when applying stucco siding from fabricated frame (tubular welded frame) scaffolding without the frames and panels being braced by cross, horizontal, or diagonal, or a combination thereof, which would have secured vertical members together laterally.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that tubular welded frames and panels are braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally.
