29 CFR 1910.137(c)(2)(viii): 29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests in accordance with Tables I-4 and I-5: (a) The Mennel Milling Company, 320 Findlay Street, Fostoria, Ohio: On or about September 25, 2014, the employer did not ensure that the protective electrical gloves used by an employee were tested every 6 months. The Salisbury gloves were tested on August 13, 2013. While replacing an electric motor on the discharge conveyor on bin #134, the employee used a Fluke Model T5-600 meter to verify that the 480 volts AC, 3 phase circuit was de-energized. The employee was at risk of serious injury from electrical shock hazards.

29 CFR 1910.335(a)(1)(ii): Protective equipment was not maintained in a safe, reliable condition and/or was not periodically inspected or tested: (a) The Mennel Milling Company, 320 Findlay Street, Fostoria, Ohio: On or about September 25, 2014, the employer did not ensure that the protective electrical gloves were inspected and tested every 6 months. The Salisbury gloves were tested on August 13, 2013 and had been in continual use since that date. While using a Fluke model T5-600 meter to verify that the 480 volts, 3 phase, circuit to bin #134 discharge conveyor motor was de-energized, the employee was at risk of serious injury from electrical shock hazards.
