(a) Location: 12 Parsons Hill Road, Wenham MA, 01984   On or about 10/2/2014, a fall protection anchor was not secured properly to the peak of the roof, and employees were not protected from falling when using a PFAS system which did not have an anchor point of supporting at least 5000 pounds.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   (a) Location: 12 Parsons Hill Road, Wenham MA, 01984   On or about 10/2/2014, employees working from a scaffold greater than ten feet above a lower level were not protected from falling.  A.C. Castle Construction Co. Inc., Daryl J. Provencher, dba Provencher Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(I), which was contained in OSHA inspection number 314963240, 931043, citation number 01, item number 011 & 003b and was affirmed as a final order on (4/6/2011 & 10/15/2013), with respect to a workplace located at 5 Collins Street, Danvers MA, 01923 & 103 Tremont Street Peabody MA, 01960.
