29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:   a) Side and roof of residence located at 1540 Silver Mist Circle, Powder Springs, Georgia. Employees, who were not wearing eye protection while using pneumatic nail guns to install siding were exposed to eye injuries.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:    a) Side of residence located at 1540 Silver Mist Circle, Powder Springs, Georgia. An employee was exposed to a fall hazard of 12-feet 3-inches as scaffold working platforms, greater than 10-feet in length, extended over supports by more than 18-inches. Access to the cantilevered portion was not blocked and the platform was not able to support the employee.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:   a) Side of residence located at 1540 Silver Mist Circle, Powder, Springs, Georgia. An employee working from pump jack scaffold working platforms was exposed to a fall hazard of 12-feet 3-inches as none of the supporting poles were bearing on a firm foundation, base plates and mud sills. The footing of the scaffold was not level, sound or rigid.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:   a) Side of residence located ay 1540 Silver Mist Circle, Powder Springs, Georgia. An employee working from a pump jack scaffold working platform was exposed to a fall hazard of 12-feet 3-inches as no guard rail, or safety net system had been installed to protect the open ends and edges of the working platform. Additionally, the exposed employee was not wearing a personal fall arrest system.

29 CFR 1926.452(j)(2): Poles of pump jack scaffold(s) were not secured to the structure by rigid triangular bracing, or equivalent, at the bottom, top and other points as necessary:   a) Side of residence located at 1540 Silver Mist Circle, Powder Springs, Georgia. The top of the pump jack scaffold system poles were securely braced to the structure. However, the bottoms of the poles, which were not braced to the structure to prevent movement or dislocation, presented a fall hazard of 12-feet 3-inches.

An employee installing siding from a garage roof was exposed to fall hazards of approximately 10-feet 11-inches as the edges of the roof were not protected and the employee was not wearing a personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) Front and side of garage of residence located at 1540 Silver Mist Circle, Powder Springs, Georgia. An employee was exposed to fall hazards while mounting and dismounting aluminum extension ladders. The side rails of the ladders did not extend at least 3-feet above the upper landing, a low sloped, 4 in 12, unprotected roof, which was 10-feet 11-inches above ground level.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a) Side of residence located at 1540 Silver Mist Circle, Powder Springs, Georgia. An employee was observed while installing siding from an elevated working platform of a pump jack scaffold. The working platform was approximately 12-feet 3-inches above the lower, ground, level. The employee had not received training to provide him with the ability to recognize and minimize the hazards associated with the type of scaffold being used.
