29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: Along the southwest corner of the construction site, on or about October 01, 2014, an employee was exposed to a struck-by hazard from loose rocks on the excavation face when in the trench measuring greater than 10 feet in depth, 17 feet in width, and 27 feet length, while moving soil away from a draining culvert using a shovel.
