29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    On or about October 1, 2014, employees were exposed to hazards such as but not limited to: struck-by from soil and equipment from trenching activities while not being trained on regulations that would apply in order to reduce the possible work environment hazards.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:    Along the southwest corner of the construction site, on or about October 01, 2014, an employee was exposed to a struck-by hazard from loose rocks on the excavation face when in the trench measuring greater than 10 feet in depth, 17 feet in width, and 27 feet length, while moving soil away from a draining culvert using a shovel.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    Along the southwest corner of the construction site, on or about October 01, 2014, an employee was exposed to a struck-by hazard, when inside a trench that measured greater than 10 feet in depth, 17 feet in width, and 27 feet in length with no protective system, while moving soil away from a draining culvert using a shovel.
