29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a)  101 South 25th Street, Harrisburg, PA - On or about October 1, 2014, an employee, repairing concrete curb and sidewalk damage, was not protected from flying particles while dry cutting the concrete with a STIHL TS 500i cut-off saw because safety glasses and/or face shields were not used.        b)  101 South 25th Street, Harrisburg, PA - On or about October 1, 2014, an employee, repairing concrete curb and sidewalk damage, was not protected from the environmental hazard of traffic flow while working in the roadway because high-visibility safety apparel, which meets the Performance Class 2 or 3 requirements of the ANSI/ISEA 107 - 2004 publication entitled American National Standard for High-Visibility Safety Apparel and Headwear, or equivalent revisions, was not provided for use.      c)  101 South 25th Street, Harrisburg, PA - On or about October 1, 2014, an employee, repairing concrete curb and sidewalk damage, was not protected from the chemical hazard of low pH while opening, mixing, pouring, and forming the Rapid Set Professional Grade concrete mix because chemical-resistant gloves were not used.      d)  101 South 25th Street, Harrisburg, PA - On or about October 1, 2014, an employee, repairing concrete curb and sidewalk damage, was not protected from respirable silica dust, while dry cutting the concrete with a STIHL TS 500i cut-off saw. Initially a dust mask was not being used, and later a dust mask was used incorrectly.        Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
