29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:       a) Shop area, Marathon horizontal bailer, 3000 Seventh Avenue, Suite 300, Altoona, Pennsylvania 16602:  Observed on or about September 25, 2014, employees standing on the upper frame of the Marathon brand horizontal baler (model number SE-503042720, serial number 86169) were not protected from falling off of the machine.  Employees standing on the horizontal baler were exposed to a fall hazard off approximately 5 feet to the concrete floor below.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Shop area, Marathon horizontal bailer, 3000 Seventh Avenue, Suite 300, Altoona, Pennsylvania 16602:  Observed on or about September 25, 2014, the Marathon brand horizontal baler (model number SE-503042720, serial number 86169) was operated with out a guard.  A screen used to guard the RAM area of the baler was removed.  Employees operating the horizontal baler were exposed to caught-by and crushed-by hazards.    b) Shop area, Marathon horizontal bailer, 3000 Seventh Avenue, Suite 300, Altoona, Pennsylvania 16602:  Observed on or about September 25, 2014, the Harmony Enterprises brand horizontal baler was operated without proper guarding.  The baler door safety interlock switch was not operable and the baler door would not latch.  Employees operating the horizontal baler were exposed to caught-by and struck-by hazards.      c) Shop area, #2 Philadelphia Tram Rail brand downstroke baler, 3000 Seventh Avenue, Suite 300, Altoona, Pennsylvania 16602:  Observed on or about September 25, 2014, the #2 baler was not properly guarded.  The baler was operated with a missing a slide gate.  Employees operating the #2 baler were exposed to caught-by and crushed-by hazards.    d) Shop area, #1 Philadelphia Tram Rail brand downstroke baler, 3000 Seventh Avenue, Suite 300, Altoona, Pennsylvania 16602:  Observed on or about September 25, 2014, the #1 baler was not properly guarded.  The baler slide gate would not latch.  Employees operating the #2 baler were exposed to caught-by and crushed-by hazards.

1910.305(g)(1)(iv)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   a) Shop area, behind Marathon horizontal bailer, 3000 Seventh Avenue, Suite 300, Altoona, Pennsylvania 16602:  Observed on or about September 25, 2014, a flexible cord plugged into a 120 volt outlet box located on a yellow beam behind the Marathon horizontal bailer was used as a substitute for the fixed wiring of the structure.  The cable was exposed to the movement of forklifts and forklift loads which could damage or weaken the cable.  Employees were exposed to electric shock and burn hazards.

The panels were used to supply power to the downstroke balers and the Xt-200 EPS Recycling Machine.
