29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about October 1, 2014, employees engaged in installing steel beams on a commercial construction project climbed and walked along the steel beams and roof areas without fall protection exposing them to an unprotected fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about October 1, 2014, and at times prior thereto, employees engaged in steel erection activities used "A" framed ladders in the closed position to gain access to the upper levels and steel beams. The employer did not ensure the ladders were used according to the manufacturer's requirements and design exposing the employees to fall hazards.
