29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    a. On or about October 1, 2014, at the above addressed jobsite, employees accessing and egressing the roof were exposed to falls of approximately 9 feet as the ladder in use, which was set along the sloped side of the roof, only extended 1-1.5 feet above the roof line.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:    a. On or about October 1, 2014, at the above addressed jobsite, employees performing residential roofing work atop the 4/12 pitched roof were exposed to falls of approximately 9-9.4 feet due to the lack of fall protection     Reyes Roofing LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 110212, citation number 1, item number 1a and was affirmed as a final order on March 15, 2012, with respect to a workplace located at 2527 W. Pershing in Chicago, IL 60632.
