29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  The employer does not protect employees engaged in a steel erection activity who are on a walking/working surface with an unprotected side or edge more than 15 feet above a lower level.  This violation was observed on or about October 3, 2014, on the west side of the building where employees were exposed to fall hazards of more than 15 feet above a lower level when attaching metal studs to an unprotected edge without a fall protection system.   Pursuant to 29 C.F.R. 1903.19, within 10 (ten) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 15 feet or more with unprotected side and edges.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed, nor was there a grasping device, such as a grabrail to assist the employees in mounting the ladder.  The employer does not ensure that portable ladders were used for access to an upper landing surface with the ladder side rails extending at least 3 feet above the upper landing surface being accessed.  This violation was observed on or about October 3, 2014, on the west side of the building,  where employees were exposed to a fall hazard when using a portable ladder to access a roof without the ladder side rails extended at least 3 feet above the landing.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees using portable ladders are protected from falling by ensuring the ladders side rails extend at least 3 feet above the upper landing surface being accessed.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  The employer does not ensure that non-self supporting ladders used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder.  This violation was observed on or about October 3, 2014, on the west side of the building where employees were exposed to fall hazards when utilizing an extension ladder that was set up at an incorrect angle to access the roof of building.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees using portable ladders are protected from falling by ensuring the ladders side rails extend at least 3 feet above the upper landing surface being accessed.
