29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a.  On or about October 1, 2014, an employee was performing tree trimming work using a truck mounted articulating bucket lift did not wear personal restraint system with a lanyard fastened to the basket, exposing himself to a 15 feet fall hazard.
